Registration and Forms

• Registration Procedures: All registration must be completed through the rSchoolToday registration link above and turn in the appropriate physical paperwork to the CHS Athletic Office.
• In order to participate in school-sponsored athletics, students registering for athletic participation must have a current physical examination (Health History Form and Physical Form) on file. To participate on a school-sponsored interscholastic team, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide the health history update form (Health History Update Form) signed by the student’s parent or guardian. These forms are needed BEFORE a student is authorized to participate in practice/tryouts. Forms that are COMPLETELY filled out and signed/stamped by your physician, can be turned into the CHS Athletic Office. To ensure clearance for participation, physical forms and the health history update form should be completed and turned in to the Athletic Office by the dates listed below. See Registration Deadline Dates. Please have the physician complete medical information on the forms linked above. These (NJDOE) forms are the ONLY forms that can be accepted.

The physical must be current and valid on the first day of official practice.

Registration Deadlines Dates
Fall sports physicals are due on the third Monday in July 15, 2024
Winter sports physicals are due on the third Monday in October 21, 2024
Spring sports physicals are due on the third Monday in February 17, 2025
