Dear Parents/Guardians of SOMSD Elementary School and Sixth Grade Students,
We would like to remind you of the opportunity and importance of having your children immunized against COVID-19. Currently, children 5 -11 years old are now eligible for the Pfizer COVID-19 Vaccine, a 2-dose series given three weeks apart.
We would appreciate having a copy of your child’s COVID vaccination card with BOTH vaccinations. This will help us identify students who may NOT have to quarantine*.
- *Note: Under the current NJDOH guidance, students who are fully vaccinated for COVID-19 may continue in-person learning even if they are identified as close contacts or after they have traveled outside NJ, NY, PA, DE, or CT (as long as they remain without symptoms).
- Fully vaccinated for a 5-11-year-old means: a) you are two weeks out from a second Pfizer shot; or b) you have been clinically recovered from COVID-19 in the past three months.
Please answer the questions and upload proof of your child’s TWO vaccinations via the SOMSD Student Vaccination Google Form (the form can also be found on the District homepage under the Fall 2021 Return to School Tab): https://bit.ly/SOMSDSVS
Important notes:
- Even if you have given a copy to your school nurse, this form must be submitted via this link.
- If you have already uploaded your child’s vaccinations onto the link, you do not have to resubmit.
The District is also pleased that we have been able to add bi-weekly onsite mobile testing to the plethora of local options available to members of our community. COVID-19 testing and vaccination opportunities can be found here: bit.ly/C19Test_Vaccine
