We would like to remind you of the opportunity and importance of having your child vaccinated against COVID-19. Children 5 years and older are eligible for the COVID-19 Vaccine.

We would appreciate having a copy of your child's COVID vaccination card showing they have received BOTH vaccinations (or ONE J&J vaccine). This will help us identify fully vaccinated students who may NOT have to quarantine.*

*NOTE: Under the current NJDOH guidance, students who are fully vaccinated** for COVID-19 vaccination do not have to quarantine and may continue in-person learning even if they are identified as close contacts or after they have traveled outside NJ, NY, PA, DE or CT (as long as they remain without symptoms).

**Fully vaccinated means: a) you are are two weeks out from a second Pfizer or Moderna shot; or b) you are two weeks out from the one Janssen/Johnson & Johnson (J&J) vaccine; or c) you have been clinically recovered from COVID-19 in the past three months.

Please answer the questions and upload proof of your child's TWO vaccinations (or ONE J&J vaccine) on the google form attached. Please fill out one form for EACH student attending schools in the district ages 5 and up. NOTE*** Even if you have already submitted your child's vaccine card to the school nurse, please upload onto this form.
