On left hand side is Athletic Forms which you should download.

You’ll need:  1.   Doctor/Physician Form (Part B – Needed once per school year -   Annual Athletic Pre-participation Physical Evaluation Form which your doctor needs to fill out and must be current for the season you want to play.

2.  Parent Health History Questionnaire & Permission Form - (Part A - needed for every sport you participate in.  Submit before each season.  You and parent/guardian fill this out)

3.  Concussion Form – needed once per school year

4.   Steriod Form – needed once per school year

5.  Manager form - (only needed if you want to be a manager)
